Christ's Church of Oronogo Terms and Agreement

TRAVEL TERMS:
As the parent or legal guardian of the above named minor, I give my permission for him or her to participate in the 5•6 Camp at Maranatha Bible Camp June 22 - 25, 2025. I give my permission for the above named minor to ride the bus to and from Maranatha Bible Camp.

I understand, it is my responsibility to have my child at Christ's Church of Oronogo Student Ministry Center East by 1:00pm on Sunday, June 22, 2025. I further understand that if the transportation leaves prior to my arrival that there will be no refund of my registration fee.

The expectation is that each person attends 5•6 Camp in its entirety and that they will travel to and from Maranatha Bible Camp with the group.

I agree to be at Christ's Church of Oronogo, Student Ministry Center East on Wednesday, June 25th at 3:30pm to pick up my child at the completion of 5•6 Camp.
**If circumstances change our arrival time, you will be notified by Christ's Church of Oronogo Student Ministry Staff.

I understand and grant permission for my child to participate in activities during the event including zip lining, archery, axe throwing, swimming, inflatables, as well as other indoor and outdoor recreational games and activities.

I understand that there are inherent risks involved in these activities and hereby release Christ's Church of Oronogo, its staff, and volunteers from responsibility and liability for any injury or illness sustained during this event.

I authorize the minister, adult leader, or sponsor of this event, or any Christ's Church of Oronogo staff member, in the event I cannot be reached by phone, to give consent to a physician and or hospital for emergency medical or surgical treatment. It is understood that I will assume any financial responsibility for any expense that may be incurred for said emergency treatment.

BEHAVIORAL TERMS:
I understand that if attendee should exhibit behavior that is inappropriate, at the discretion of the Christ's Church of Oronogo staff, they will be sent home from Maranatha Bible Camp. Transportation will be the responsibility of the attendee's parent or guardian and the registration fee will not be refunded.

I authorize Christ's Church of Oronogo to use photographs and/or video footage of my child for future promotional material.

Maranatha Bible Camp Participant Release Form

Camper Behavior:
I recognize this is a Christian camp, that the Bible will be studied, and that conduct will be expected that is consistent with Biblical values.

Medical Treatment:
To the best of my knowledge the participant named on this form is, physically and emotionally able
to take part in the camp program. In the event of a medical emergency, I give permission to the volunteer medical
person selected by the camp management to do whatever is necessary for the health of the participant named on this
form. I understand the Maranatha Bible Camp staff have preventative measures in place to reduce the spread of
communicable diseases including COVID-19 but they cannot guarantee that an event participant will not become
infected with COVID-19 or other communicable disease. I hereby release the camp from any responsibility other than
normal supervision and care. In case of an illness or accident, I will not hold Maranatha Bible Camp or its staff members,
management, or officers liable.

Photo Permissions:
I give my permission for appropriate photographs and video to be used for future Maranatha Bible Camp promotional purposes.

Maranatha Recreation Waiver

Recreation/Participation Waiver:
I understand that participants attending Maranatha Bible Camp may be offered an opportunity to participate in multiple recreation options including but not limited to: Swimming, Riding Zip Lines, Archery, Human Soccer, Axe Throwing and playing on Inflatables. I fully understand and acknowledge that 1) risks and dangers exist in the participation in and use of these recreation options, 2) that participating is such activities and/or use of such equipment may result in injury or illness including but not limited to bodily injury, disease, strains, fractures, partial and/or total paralysis, eye injury, blindness, heat stroke, heart attack, death or other ailments that could cause serious disability; 3) and by giving permission for the person named in this registration to participate in these activities and use this equipment, I assume all risks, dangers, and responsibility for any losses and/or damages incurred while
participating in these activities. I release Maranatha Bible camp and any entities they contract with for specialized
recreation from all liability. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of this organization, its employees, agents, and representatives, whether an injury or infection occurs before, during, or after participation in any such program, service or activity.

Medical Information

IMPORTANT INFORMATION FROM MARANATHA BIBLE CAMP:

  • We do NOT use PEANUT products in our kitchen however, some items may have been packaged by the manufacturer in facilities that also package peanuts. We are glad to let you read any of our food labels. Additionally, some pre-packaged canteen items do contain peanuts.
  • Our kitchen is gluten friendly. We routinely keep several gluten free products in stock. Please note that we do our best to avoid cross contamination but do not have a separate kitchen for cooking of gluten free items.
  • Vegans who do not consume dairy or eggs should plan to bring protein supplements as we are not equipped to provide for a strict vegan diet.

Over-The-Counter Medication Approval

Over-the-counter medications will be available through the camp nurse.  These include:  ibuprofen, acetaminophen, naproxin, cold medicine, allergy medicine, Dramamine, Immodium, Pepto, Tums, antibiotic cream, hydrocortizone cream, sunscreen and aloe vera.

You are required to pay the deposit of $100.00 to secure your registration.  You may, however, choose to pay the full amount at this time if you would like.  If you choose to pay only the deposit at this time, the remaining balance will be automatically deducted from your account on June 9th.
